In an emergency, during a new appointment, or when helping a family member, having the right medical information in one place can make care easier and safer.

What to include

  • Full legal name, date of birth, phone, address, and emergency contact.
  • Current medications, including dose and how often you take them.
  • Allergies and reactions.
  • Medical conditions.
  • Surgeries and hospitalizations.
  • Doctors, specialists, and pharmacy.
  • Important documents such as healthcare power of attorney, living will, advance directive, or DNR order if applicable.

Where to keep it

Consider keeping copies in your wallet or purse, on your phone, with a trusted family member, in your patient portal, or somewhere visible for emergency responders if appropriate.

Keep it updated

Update it when medications, diagnoses, insurance, emergency contacts, or medical wishes documents change.
